Before you begin I will tell you the questionnaire is lengthy. I would like you to know that there are no right or wrong answers and I ask that you please fill it out truthfully and completely. My intent is not to be invasive or offensive; I only want to insure I am finding the BEST possible homes for my puppies as well as ensure that I am providing you with the best puppy for your family.
